{"id":39492,"date":"2026-05-28T14:40:07","date_gmt":"2026-05-28T14:40:07","guid":{"rendered":"https:\/\/pickbydoc.com\/?p=39492"},"modified":"2026-05-28T14:40:10","modified_gmt":"2026-05-28T14:40:10","slug":"in-a-vaccine-skeptical-california-county-a-potential-playbook-to-contain-measles","status":"publish","type":"post","link":"https:\/\/pickbydoc.com\/?p=39492","title":{"rendered":"In a Vaccine-Skeptical California County, a Potential Playbook To Contain Measles"},"content":{"rendered":"<p> <br \/>\n<\/p>\n<div>\n<p>James Mu had braced for the call that came in late January.<\/p>\n<p>A patient from his rural Northern California county <a href=\"https:\/\/www.shastacounty.gov\/media\/79981\" target=\"_blank\" rel=\"noopener\">had measles<\/a>, a disease so rare there that many physicians have never treated a case.<\/p>\n<p>While California has some of the strictest vaccine laws in the country, conservative Shasta County\u2019s approach during the covid pandemic stood in stark contrast with the state\u2019s guidance. Its local leaders opposed masking and vaccine mandates, and <a href=\"https:\/\/www.redding.com\/story\/news\/2022\/05\/03\/shasta-supervisors-vote-fire-health-officer-ramstrom\/9635663002\/\" target=\"_blank\" rel=\"noopener\">they ousted<\/a> the county public health officer, who had sought to enforce those state policies and other safety measures.<\/p>\n<p>A potential measles outbreak had \u201calways been in my mind,\u201d said Mu, an outspoken family physician who was <a href=\"https:\/\/www.latimes.com\/california\/story\/2023-10-19\/la-me-covid-vaccine-public-health-officer-shasta-county-california\" target=\"_blank\" rel=\"noopener\">among local doctors<\/a> to sign a <a href=\"https:\/\/web.archive.org\/web\/20231018170347\/https:\/www.covid19publicforum.com\/letter\" target=\"_blank\" rel=\"noopener\">2022 letter<\/a> opposing covid vaccine mandates. But Mu, the county\u2019s current public health officer, said that when his department identified the first local measles case, it acted decisively: \u201cWe forgot about fear.\u201d<\/p>\n<p>They went to work, he and his team said, to painstakingly retrace the steps of <a href=\"https:\/\/www.shastacounty.gov\/health-human-services\/page\/pr-additional-shasta-county-measles-case-confirmed-exposure-sites\" target=\"_blank\" rel=\"noopener\">nine people<\/a> sickened with measles, contacting more than 600 people who may have been exposed at Costco, a sushi restaurant, sporting events, a school, or a healthcare clinic. Just one of the nine contracted measles from one of those locations, while the others were characterized by the public health department as \u201cclose contacts.\u201d<\/p>\n<p>Two and a half months later, the Shasta County public health department had declared the measles outbreak over. Infectious disease experts say the rapid response executed in the mostly rural, vaccine-hesitant county offers a playbook for public health officers across the nation who are struggling to keep the highly contagious virus from spreading.<\/p>\n<p>\u201cTo me, the story of Shasta is one of hope,\u201d said Peter Chin-Hong, an infectious disease specialist at the University of California-San Francisco.<\/p>\n<figure class=\"wp-block-image alignwide size-large\"><figcaption class=\"wp-element-caption\">Downtown Redding, California, the seat of Shasta County. (iStock\/Getty Images)<\/figcaption><\/figure>\n<p>After more than a year of ongoing cases, measles has sickened more than <a href=\"https:\/\/www.cdc.gov\/measles\/data-research\/index.html\" target=\"_blank\" rel=\"noopener\">4,000 people<\/a> in the U.S., according to the Centers for Disease Control and Prevention. For the first time in two decades, the U.S. is poised to lose its measles elimination status, a designation signaling that outbreaks are rare and rapidly contained.<\/p>\n<p><a href=\"https:\/\/epi.utah.gov\/measles-response\/\" target=\"_blank\" rel=\"noopener\">Utah had confirmed<\/a> 673 measles cases as of late May while <a href=\"https:\/\/dph.sc.gov\/news\/tuesday-measles-update-dph-reports-no-new-measles-cases-friday-outbreak-total-remains-997-0\" target=\"_blank\" rel=\"noopener\">South Carolina<\/a> had seen at least 997, according to their state health departments. <a href=\"https:\/\/www.cdph.ca.gov\/Programs\/CID\/DCDC\/Pages\/Immunization\/measles.aspx\" target=\"_blank\" rel=\"noopener\">California had confirmed<\/a> 74 cases.<\/p>\n<div class=\"wp-block wp-block-kff-shared-newsletter  wp-block-kff-shared-newsletter--background-white\" data-type=\"kff-shared\/newsletter\" data-align=\"center\">\n<div class=\"wp-block-kff-shared-newsletter__container\">\n<div class=\"wp-block-kff-shared-newsletter__content\">\n\t\t\t<img decoding=\"async\" src=\"https:\/\/kffhealthnews.org\/wp-content\/plugins\/kff-shared\/dist\/\/images\/newsletter-icon.png\" alt=\"Newsletter Icon\" class=\"wp-block-kff-shared-newsletter__img\"\/><\/p>\n<div class=\"wp-block-kff-shared-newsletter__text\">\n<h4 class=\"newsletter__title\">\n\t\t\t\t\tEmail Sign-Up\t\t\t\t<\/h4>\n<p class=\"newsletter__description\">\n\t\t\t\t\tSubscribe to KFF Health News&#8217; free weekly newsletter, &#8220;The Week in Brief.&#8221;\t\t\t\t<\/p>\n<\/p><\/div>\n<\/p><\/div>\n<\/p><\/div>\n<\/div>\n<p class=\"has-heading-5-font-size\"><strong>Critical Rapid Response<\/strong><\/p>\n<p>In late January, when Shasta County identified the first case, Mu gathered with more than a dozen communicable-disease nurses, epidemiologists, and emergency and community relations staffers for an \u201cinitial threat assessment meeting.\u201d<\/p>\n<p>Measles is an <a href=\"https:\/\/www.cdc.gov\/measles\/causes\/index.html\" target=\"_blank\" rel=\"noopener\">airborne pathogen<\/a> that can linger in a room for two hours after an infected person leaves, so on-call nurses and responders faced a daunting task figuring out exactly when the patient was infectious and where they had been.<\/p>\n<p>\u201cEverything is about speed \u2014 speed in identifying the person and finding the sites where measles were occurring,\u201d Chin-Hong said. \u201cIf you keep it down to a few cases, it\u2019s much easier. If you wait just a little bit longer, those people would have been in contact with a lot more people.\u201d<\/p>\n<p>Roughly 9 in 10 unvaccinated people exposed to the virus become infected. All nine of Shasta County\u2019s confirmed cases were people who were unvaccinated or had unknown vaccination status, according to the county\u2019s public health department. Before the department called families who may have been exposed, county nurses sometimes enlisted school principals, church staff, clinic managers, or others to make first contact, said Daniel Walker, the county\u2019s supervising epidemiologist.<\/p>\n<p>Erika Piper, the head of Redding Christian School in Palo Cedro, talked to school families wary of requests by public health officials \u2014 and government in general \u2014 to provide immunization records or other personal information. She said she also had tough but respectful conversations with families to ensure exposed, unvaccinated kids stayed home from school, so their community could abide by public health guidance calling for <a href=\"https:\/\/publichealth.jhu.edu\/2026\/what-is-a-measles-exposure-alert\" target=\"_blank\" rel=\"noopener\">21 days of isolation<\/a>.<\/p>\n<p>\u201cI would say to them: \u2018That\u2019s totally fine. You have a choice. You\u2019ve made your choice. But there are still consequences to the choices we make,\u2019\u201d Piper said, referring to families who had opted not to vaccinate their children. \u201c\u2018And so you can either be a willing helper and a partner with me in this, and we can make it work and get through it, or you can battle me on it. But either way, you can\u2019t be in school.\u2019\u201d<\/p>\n<p>She allowed work to be sent home to quarantined students and personally took daily attendance at the school to help ensure health guidelines were met.<\/p>\n<p>The California Department of Public Health assisted with case investigation by making calls to exposed people at the county\u2019s request and deployed a covid-era phone system, CalCONNECT, that automates symptom monitoring for exposed contacts.<\/p>\n<p>Shasta officials warned people not to be wary of calls from contract tracers using a 279 area code, worrying they would dismiss them as scams.<\/p>\n<p class=\"has-heading-5-font-size\"><strong>Delicate Conversations<\/strong><\/p>\n<p>In Shasta County, the measles vaccination rate is just below the <a href=\"https:\/\/www.cdc.gov\/global-measles-vaccination\/about\/index.html#:~:text=Recommendations,and%20support%20measles%20elimination%20activities.&amp;text=Measles%20is%20one%20of%20the%20most%20contagious%20human%20diseases.,Strong%20and%20...\" target=\"_blank\" rel=\"noopener\">95% threshold<\/a> for community-level protection, but in pockets of the community the rates are lower and vary widely, according to <a href=\"https:\/\/public-usa.mkt.dynamics.com\/api\/orgs\/24ac4e14-c54e-4da4-af0e-09805e16f9b9\/r\/JQcQCGXsZ0CCkBU5G34BAAYAAAA?msdynmkt_target=%7B%22TargetUrl%22%3A%22https%253A%252F%252Fwww.cdph.ca.gov%252FPrograms%252FCID%252FDCDC%252FCDPH%252520Document%252520Library%252FImmunization%252F2024-25KindergartenSummaryReport.pdf%22%2C%22RedirectOptions%22%3A%7B%225%22%3Anull%2C%221%22%3Anull%7D%7D&amp;msdynmkt_digest=Xpm0MGm6zblpKzGmkIJslAIb%2Bg%2FWpSh9y9P2gOJOiLo%3D&amp;msdynmkt_secretVersion=a3955a744d9049f88dd8bdc1907bf2bf\" target=\"_blank\" rel=\"noopener\">state data<\/a>. And in those vulnerable places, an outbreak can spread.<\/p>\n<p>For example, more than a quarter of Shasta schools had rates below 95% in 2024-25, according to the latest state data available. Several were below 90%. Although Redding Christian School reported a kindergarten measles vaccination rate at or above 95% in 2024-25, it was 87.8% three years earlier.<\/p>\n<p>When it came to talking to people who had been exposed to measles, Sharayne Loomis, a supervising public health nurse on Shasta\u2019s communicable-disease team, described the department\u2019s approach as \u201cmeeting people where they are.\u201d That included nonjudgmental conversations that supported residents regardless of their stance on vaccination, Loomis said.<\/p>\n<p>Mu said the same philosophy extended across the county\u2019s health agencies, but he publicly <a href=\"https:\/\/shastascout.org\/this-is-what-weve-been-preparing-for-shasta-public-health-officer-dr-james-mu-responds-to-countys-first-measles-case\/\" target=\"_blank\" rel=\"noopener\">cautioned against<\/a> \u201cmeasles parties,\u201d gatherings where unvaccinated children are intentionally exposed to build immunity. And he spoke against receiving high doses of vitamin A without medical supervision. Vitamin A has circulated as a measles treatment in vaccine-skeptical communities and was endorsed last year by Health and Human Services Secretary Robert F. (Shasta County)<\/figcaption><\/figure>\n<p>Some community members said Mu\u2019s department could have been more proactive before the outbreak, imploring him to emphasize the importance of vaccination in public messaging.<\/p>\n<p>\u201cClearly, when the situation was known to be coming into our communities, that would have been a time to advise for vaccines,\u201d Steve Kahn told county supervisors at their February board meeting. \u201cI think he was negligent in that.\u201d<\/p>\n<p>For years, public health has been a political flash point for the region. The Board of Supervisors fired the previous public health officer, Karen Ramstrom, in May 2022 after <a href=\"https:\/\/www.change.org\/p\/people-of-shasta-county-have-dr-karen-ranstrom-at-shasta-county-hhsa-resign-for-abuse-of-power\" target=\"_blank\" rel=\"noopener\">pushback from residents<\/a> upset with her enforcement of state covid rules.<\/p>\n<p>In an effort to reach vaccine-hesitant Californians, state officials have been working in a coalition called Public Health for All Californians Together and through an effort nicknamed <a href=\"https:\/\/www.cdph.ca.gov\/Programs\/OPP\/Pages\/publichealth4all\/phnix.aspx\" target=\"_blank\" rel=\"noopener\">Project Stethoscope<\/a> that uses social media monitoring and other research to tailor messaging to skeptical viewers.<\/p>\n<p>Erica Pan, director of the California Department of Public Health, said the state is preparing for measles to possibly surge when it hosts World Cup soccer matches starting in June, as well as with increased summer travel.<\/p>\n<p>But when it comes to mitigating an outbreak in a community, public health officials say, residents \u2014 especially those skeptical of vaccines \u2014 need to hear from the people they know.<\/p>\n<p>\u201cTrust is very important for us,\u201d Mu said. \u201cIt is critical in getting people to follow our guidance, especially during an outbreak.\u201d<\/p>\n<\/div>\n<p><br \/>\n<br \/><a href=\"https:\/\/kffhealthnews.org\/public-health\/measles-outbreak-contained-vaccine-skeptical-california-shasta-county\/\" target=\"_blank\" rel=\"noopener\">Source link <\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>James Mu had braced for the call that came in late January.
