To suppress daily aches in her back and hips, her doctor prescribed opioid painkillers.<\/p>\n<p>They worked for a bit. But by 2011, Warfield struggled to walk.<\/p>\n<p>And \u201cby that time, I was addicted,\u201d said Warfield, now living in Shelbyville, Kentucky.<\/p>\n<p>After she lost her health insurance, Warfield started buying pills on the street. She tried to quit several times, but the debilitating withdrawal \u2014 so bad she couldn\u2019t get out of bed, she said \u2014 kept driving her back to drug use.<\/p>\n<p>Until last year.<\/p>\n<p>Through her church, Warfield learned about the NET device. It\u2019s a cellphone-sized pack connected to gel electrodes placed near the ear that deliver low-level electrical pulses to the brain.<\/p>\n<p>\u201cOnce I got set up on the device, within 30 minutes, I didn\u2019t have any cravings\u201d for opioids, Warfield said.<\/p>\n<p>After three days on the device in August, she stopped using drugs altogether, she said.<\/p>\n<figure class=\"wp-block-image alignwide size-full\"><figcaption class=\"wp-element-caption\">After using the NET device for three days in August, Michelle Warfield says she stopped using or craving drugs.<span class=\"photo-credit\">(Aneri Pattani\/KFF Health News)<\/span><\/figcaption><\/figure>\n<p>Warfield\u2019s treatment was paid for with her county\u2019s opioid settlement dollars \u2014 money from pharmaceutical companies accused of fueling the overdose crisis.<\/p>\n<p>State and local governments nationwide are receiving <a href=\"http:\/\/www.kffhealthnews.org\/opioid-settlements\" target=\"_blank\" rel=\"noopener\">billions in opioid settlement cash<\/a> over nearly two decades and are meant to spend it treating and preventing addiction.<\/p>\n<p>Warfield wants them to allot a good chunk to the NET device, which costs counties about $5,500 a person. The pitch is gaining traction. <a href=\"https:\/\/relief.netrecovery.net\/\" target=\"_blank\" rel=\"noopener\">NET Recovery<\/a>, which makes the device, said it has signed\u00a0about\u00a0$1.2 million in contracts with\u00a0more than a dozen counties and cities\u00a0in Kentucky.\u00a0<\/p>\n<p>But some researchers and recovery advocates say the company\u2019s rapid consumption of opioid dollars raises red flags. They see the NET device as the latest in a series of products that have been overhyped as the solution to the addiction crisis, preying on people\u2019s desperation and capitalizing on the windfall of opioid settlements. Many of these products \u2014 from <a href=\"https:\/\/opioidprinciples.jhsph.edu\/wp-content\/uploads\/2025\/01\/Opioid-Overdose-Reversal-Products-FAQ.pdf\" target=\"_blank\" rel=\"noopener\">high-dose overdose reversal medications<\/a> to <a href=\"https:\/\/kffhealthnews.org\/news\/article\/using-opioid-settlement-cash-for-police-gear-like-squad-cars-and-scanners-sparks-debate\/\" target=\"_blank\" rel=\"noopener\">body scanners for jails<\/a> \u2014 have little evidence to back their lofty promises. That has not stopped sales representatives from <a href=\"https:\/\/kffhealthnews.org\/news\/article\/opioid-settlement-money-corporations-cash-cow\/\" target=\"_blank\" rel=\"noopener\">repeatedly pitching<\/a> elected officials or circulating ready-made templates to request settlement money for the companies\u2019 products.<\/p>\n<p>In fact, a device similar to NET called the Bridge gained popularity several years ago, <a href=\"https:\/\/kffhealthnews.org\/news\/article\/opioid-settlement-funds-detailed-database-state-county-city-spending\/#database\" target=\"_blank\" rel=\"noopener\">receiving more than $215,000<\/a> in opioid settlement cash nationwide. But <a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2018\/05\/02\/602908208\/questions-raised-about-study-of-device-to-ease-opioid-withdrawal\" target=\"_blank\" rel=\"noopener\">serious questions arose<\/a> about the study backing its effectiveness, and the device is currently off the market.<\/p>\n<p>NET Recovery\u2019s activity \u201cfits the national trends of these industry money grabs,\u201d said <a href=\"http:\/\/linkedin.com\/in\/triciac13\" target=\"_blank\" rel=\"noopener\">Tricia Christensen<\/a>, a national expert on opioid settlements based in Tennessee. The device \u201ccould be helpful for some,\u201d she said. \u201cBut it\u2019s being sold as a silver bullet.\u201d<\/p>\n<p>This year, 237 organizations working to end overdose \u2014 including Christensen\u2019s consulting company \u2014 <a href=\"https:\/\/static1.squarespace.com\/static\/640e4d9374e80160b84e0be4\/t\/69681b4f6d597d42f1178cd7\/1768430415119\/FINAL-National-Roadmap-for-Spending-Opioid-2026.pdf#page=12\" target=\"_blank\" rel=\"noopener\">published a road map<\/a> to guide officials in charge of opioid settlement money. In it, they called the NET device an example of problematic spending on unproven treatment.<\/p>\n<div class=\"wp-block block--newsletter  \" data-type=\"kaiser-health-news\/newsletter\" data-align=\"center\" style=\"\">\n<h4 class=\"newsletter__title\">\n\t\t<a href=\"https:\/\/kffhealthnews.org\/email\/\" target=\"_blank\" rel=\"noopener\"><br \/>\n\t\t\tEmail Sign-Up\t\t<\/a><br \/>\n\t<\/h4>\n<p class=\"newsletter__description\">\n\t\tSubscribe to KFF Health News&#8217; free weekly newsletter, &#8220;The Week in Brief.&#8221;\t<\/p>\n<\/div>\n<p style=\"font-size:25px\"><strong>Treating Withdrawal or Addiction<\/strong><\/p>\n<p>The FDA has <a href=\"https:\/\/www.accessdata.fda.gov\/cdrh_docs\/pdf23\/K233166.pdf#page=5\" target=\"_blank\" rel=\"noopener\">cleared the NET device<\/a> for a specific use: reducing drug withdrawal symptoms. It has not approved the device to treat addiction.<\/p>\n<p>That\u2019s a crucial distinction, said <a href=\"https:\/\/ireta.org\/who-we-are\/our-organization\/ireta-staff\/\" target=\"_blank\" rel=\"noopener\">Eric Hulsey<\/a>, executive director of the Institute for Research, Education and Training in Addictions. It has not approved the device to treat addiction.<span class=\"photo-credit\">(NET Recovery)<\/span><\/figcaption><\/figure>\n<p>NET Recovery CEO <a href=\"https:\/\/www.linkedin.com\/in\/joe-winston-6225a41\/\" target=\"_blank\" rel=\"noopener\">Joe Winston<\/a> said the company adheres to FDA rules and advertises the device only for withdrawal management. But \u201cwe are finding that physicians are prescribing this to folks for long-term behavior based on the results of our study.\u201d<\/p>\n<p>He\u2019s referring to <a href=\"https:\/\/www.frontiersin.org\/journals\/psychiatry\/articles\/10.3389\/fpsyt.2025.1627267\/full\" target=\"_blank\" rel=\"noopener\">an October study<\/a> that he co-authored and the company funded, in which researchers followed two groups of addiction patients in Kentucky for 12 weeks. The first group received the NET device for up to seven days, while the second group received a sham treatment.<\/p>\n<p>The study found no significant difference between the groups\u2019 outcomes. Participants who got the NET device were similarly likely to use illicit drugs after treatment as those who got the fake.<\/p>\n<p>Hulsey, who was not affiliated with the study, said the takeaway is clear: \u201cThey didn\u2019t find that was effective.\u201d<\/p>\n<p>A subgroup of participants who chose to use the device for more than 24 hours consecutively, however, went on to use illicit drugs less often than other participants.<\/p>\n<p>As the researchers acknowledged in their paper, that subgroup might simply have been more motivated to engage with any form of treatment. The results don\u2019t necessarily show that the device is making a difference, Hulsey said.<\/p>\n<p style=\"font-size:25px\"><strong>Rapid Growth<\/strong><\/p>\n<p>Winston had a different take. He said the success of the subgroup is \u201cintriguing and outstanding.\u201d<\/p>\n<p>So outstanding, in fact, that the company this month is opening a brick-and-mortar location in Miami, where the device will be available to anyone who can pay $8,000 out-of-pocket. (The cost is higher for individuals than for county governments.) It has also applied for opioid settlement dollars from the state of Kentucky to conduct a larger research study and aims to bring the NET device into metro areas such as Louisville and Lexington.<\/p>\n<p>Last year, NET Recovery hired a magistrate in Franklin County, Kentucky, to head up its operations in the state. (Magistrates function as county commissioners.) <a href=\"https:\/\/www.linkedin.com\/in\/kelly-dycus-0872801a0\/\" target=\"_blank\" rel=\"noopener\">Kelly Dycus<\/a>, who is also a mental health clinician, travels to different counties, extolling the benefits of the device and encouraging officials to contract with the company.<\/p>\n<p>Her county <a href=\"https:\/\/franklincounty.ky.gov\/fiscal-court-approves-75000-contract-to-combat-opioid-epidemic\/\" target=\"_blank\" rel=\"noopener\">awarded $75,000<\/a> to NET Recovery prior to her joining the company. Moving forward, Dycus said, she would recuse herself from any contract votes in her county.<\/p>\n<p>Christensen, the national expert on opioid settlements, called Dycus\u2019 new role \u201cextremely strategic\u201d for the company and \u201can obvious conflict of interest\u201d for a public official.<\/p>\n<p style=\"font-size:25px\"><strong>Giving People Choice<\/strong><\/p>\n<p>More options for people to enter recovery is generally good, said Jennifer Twyman, who has a history of opioid addiction and now works with <a href=\"https:\/\/vocal-ky.org\/about-us\/\" target=\"_blank\" rel=\"noopener\">Vocal-KY<\/a>, a nonprofit that advocates to end homelessness and the war on drugs.<\/p>\n<p>But settlement funds are finite, she said, and when counties invest in the NET device, that leaves less money to support options like mental health treatment, housing, and transportation programs \u2014 critical for many people who use drugs.<\/p>\n<p>\u201cPeople slip through these big, huge gaps we have and they die,\u201d Twyman said, pointing to photos of dead friends that line her office wall.<\/p>\n<p>She added that people should have the option of taking medications such as methadone and buprenorphine \u2014 <a href=\"https:\/\/www.naco.org\/resource\/osc-mat\" target=\"_blank\" rel=\"noopener\">the gold standard<\/a> for treating opioid addiction. <a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/73\/wr\/mm7325a1.htm\" target=\"_blank\" rel=\"noopener\">National data shows<\/a> only 1 in 4 people with opioid addiction get them.<\/p>\n<figure class=\"wp-block-image alignwide size-full\"><img loading=\"lazy\" loading=\"lazy\" decoding=\"async\" width=\"3840\" height=\"2559\" src=\"https:\/\/kffhealthnews.org\/wp-content\/uploads\/sites\/2\/2026\/03\/Addiction-treatment-tech-05-resized.jpg\" alt=\"A woman stands indoors. prescribe them, or get transportation to appointments, Twyman said. <a href=\"https:\/\/www.statnews.com\/2024\/11\/12\/opioid-addiction-recovery-narcotics-anonymous-salvation-army\/\" target=\"_blank\" rel=\"noopener\">There\u2019s also discrimination<\/a> against those who use medications, with detractors saying they\u2019re not truly abstinent or clean.<\/p>\n<p>Companies like NET Recovery sometimes lean into that stigma, Twyman said.<\/p>\n<p>For instance, Scott County, Kentucky, jailer <a href=\"https:\/\/www.scottjailky.com\/Jailer.html\" target=\"_blank\" rel=\"noopener\">Derran Broyles<\/a> \u2014 whom the company considers a key champion for its device \u2014 <a href=\"https:\/\/www.youtube.com\/watch?v=rE31swa2eVE\" target=\"_blank\" rel=\"noopener\">said in a public meeting<\/a> to other county officials that medication treatment is just \u201cswapping one drug for another.\u201d It\u2019s a common refrain from critics that <a href=\"https:\/\/www.rstreet.org\/commentary\/safer-solutions-medication-for-opioid-use-disorder-doesnt-swap-one-drug-for-another\/\" target=\"_blank\" rel=\"noopener\">many researchers and clinicians refute<\/a>.<\/p>\n<p>Winston told KFF Health News his company is supportive of all types of recovery but that the NET device can help the \u201cunderserved population\u201d of people who don\u2019t want medication.<\/p>\n<p>Longtime addiction researcher <a href=\"https:\/\/psychiatry.med.wayne.edu\/profile\/aa2678\" target=\"_blank\" rel=\"noopener\">Mark Greenwald<\/a> has led studies for NET Recovery and consults for <a href=\"https:\/\/www.indivior.com\/our-products\" target=\"_blank\" rel=\"noopener\">Indivior<\/a>, one of the leading producers of medications for opioid use disorder. He said he sees value in both approaches. It just depends on whom you\u2019re trying to treat.<\/p>\n<p>For people injecting drugs or accustomed to high doses of fentanyl, who are more likely to return to using drugs after residential treatment, \u201cI would hesitate to recommend the device,\u201d he said. Abstinence-based approaches can <a href=\"https:\/\/news.yale.edu\/2023\/12\/19\/treating-opioid-disorder-without-meds-more-harmful-no-treatment-all\" target=\"_blank\" rel=\"noopener\">increase their risk of dying<\/a>. But for people who are \u201chighly motivated to stay abstinent,\u201d the NET device may be a good fit.<\/p>\n<p>\u201cGiving people choices is the right thing to do,\u201d he said.<\/p>\n<p style=\"font-size:25px\"><strong>Community as Part of Recovery<\/strong><\/p>\n<p>Warfield, who has not used opioids since August, credits not just the NET device with her recovery but her community too.<\/p>\n<p>\u201cIt\u2019s not a miracle cure,\u201d she said of the device. \u201cYou still have to manage your triggers, but it\u2019s easier.\u201d<\/p>\n<p>She regularly attends individual and group therapy to address childhood trauma. She\u2019s found close friends within her church and has reconnected with her daughter.
